From Screening to Action: Food Insecurity as a Driver of Chronic Disease in Rural Care Management

This article, published in Collaborative Case Management, explores how food insecurity drives chronic disease in rural communities and outlines practical strategies for case managers to move from screening to meaningful intervention. Rooted in my DrPH dissertation research, the piece uses real‑world EMR data and rural care management experience to highlight the critical role of social drivers of health (SDOH) in shaping long‑term outcomes.
